    Treatment biomarkers for ADHD: Taking stock and moving forward

    The development of treatment biomarkers for psychiatric disorders has been challenging, particularly for heterogeneous neurodevelopmental conditions such as attention-deficit/hyperactivity disorder (ADHD). Promising findings are also rarely translated into clinical practice, especially with regard to treatment decisions and development of novel treatments. Despite this slow progress, the available neuroimaging, electrophysiological (EEG) and genetic literature provides a solid foundation for biomarker discovery. This article gives an updated review of promising treatment biomarkers for ADHD which may enhance personalized medicine and novel treatment development. The available literature points to promising pre-treatment profiles predicting efficacy of various pharmacological and non-pharmacological treatments for ADHD. These candidate predictive biomarkers, particularly those based on low-cost and non-invasive EEG assessments, show promise for the future stratification of patients to specific treatments. Studies with repeated biomarker assessments further show that different treatments produce distinct changes in brain profiles, which track treatment-related clinical improvements. These candidate monitoring/response biomarkers may aid future monitoring of treatment effects and point to mechanistic targets for novel treatments, such as neurotherapies. Nevertheless, existing research does not support any immediate clinical applications of treatment biomarkers for ADHD. Key barriers are the paucity of replications and external validations, the use of small and homogeneous samples of predominantly White children, and practical limitations, including the cost and technical requirements of biomarker assessments and their unknown feasibility and acceptability for people with ADHD. We conclude with a discussion of future directions and methodological changes to promote clinical translation and enhance personalized treatment decisions for diverse groups of individuals with ADHD

    In this paper, we are presenting a data structure, which stores the given dictionary data in a hash table called PRIME , by using fundamental theorem on Arithmetic to generate a key for each dictionary word, and stores the word in the hash table based on the key. As compared to tree-based techniques PRIME table generates anagram for the given random word in O(1) time, time to construct a PRIME table depends on the number of words in the dictionary.     Professional perceptions of the care-crime connection: Risk, marketisation and a failing system

    Reporting the findings from an English study of practitioners working within multi-agency settings, this article will explore their perceptions of the factors that contribute to the criminalisation of children in care. The findings support the contention that children get into trouble as a consequence of a number of system level deficiencies, along with a defensive, risk averse approach to practice which permeates throughout children’s social care and criminal justice agencies. The impact of an under-confident and transient residential care workforce is discussed, along with the privatisation of children’s home provision. Yet while residential care was confirmed as being the more problematic environment, children in foster care were not immune to unnecessary criminalisation. Unchanged, this will continue to produce negative outcomes in terms of criminalisation, the longer term effects of which will impact the life-chances of young people for years to come

    Children in Care: Exploitation, Offending and the Denial of Victimhood in a Prosecution-led Culture of Practice

    The following article reports upon recent research, which explored the perceptions of professionals of the issues that affect the sexual and criminal exploitation of children in care, along with a discussion of the effectiveness of current responses to these issues and the challenges that professionals face. The study utilised focus groups and semi-structured interviews to gain the perspectives of thirty-six participants from across a range of agencies involved in children’s social work and youth justice from an area in the north-west of England. The findings echoed previous research by suggesting that for a number of reasons connected to their ‘looked-after’ status and deficiencies within the care system, children are particularly vulnerable to exploitation. Although attempts are being made to develop co-ordinated responses to safeguarding, there is still a long way to go in terms of focusing on vulnerability instead of aprosecution-led approach when offending occurs as a result of exploitation. It is argued how in order to ensure that good, innovative practice becomes ‘standard’ practice; further legislation is required in tandem with a greater degree of education surrounding exploitation and the dynamics inherent in these offence

    Tensor estimation for double-pulsed diffusional kurtosis imaging

    Double-pulsed diffusional kurtosis imaging (DP-DKI) represents the double diffusion encoding (DDE) MRI signal in terms of six-dimensional (6D) diffusion and kurtosis tensors. Here a method for estimating these tensors from experimental data is described. A standard numerical algorithm for tensor estimation from conventional (i.e. single diffusion encoding) diffusional kurtosis imaging (DKI) data is generalized to DP-DKI. This algorithm is based on a weighted least squares (WLS) fit of the signal model to the data combined with constraints designed to minimize unphysical parameter estimates. The numerical algorithm then takes the form of a quadratic programming problem. The principal change required to adapt the conventional DKI fitting algorithm to DP-DKI is replacing the three-dimensional diffusion and kurtosis tensors with the 6D tensors needed for DP-DKI. In this way, the 6D diffusion and kurtosis tensors for DP-DKI can be conveniently estimated from DDE data by using constrained WLS, providing a practical means for condensing DDE measurements into well-defined mathematical constructs that may be useful for interpreting and applying DDE MRI. Data from healthy volunteers for brain are used to demonstrate the DP-DKI tensor estimation algorithm. This research represents a shift towards a holistic view where evaluation of health technologies is linked to organisational practices. In this paper, we address the gaps in existing literature regarding the holistic evaluation of e-health in clinical practice. We report the results from a qualitative study conducted to gain insight into e-health in practice within an interdisciplinary healthcare domain. Findings from this qualitative study, provides the foundation for the creation of a generic measurement model that allows for the comparative analysis of health technologies and assist in the decision-making of its stakeholders

    Nutritional status and the gonadotrophic response to a polar expedition.

    Polar expeditions have been associated with changes in the hypothalamic-pituitary-testicular axis consistent with central hypogonadism (i.e., decreased testosterone, luteinising hormone (LH), and follicle stimulating hormone (FSH)). These changes are typically associated with body mass loss. Our aim was to evaluate whether maintenance of body mass during a polar expedition could mitigate against the development of central hypogonadism. Male participants (n = 22) from a 42-day expedition (British Services Antarctic Expedition 2012) volunteered to take part in the study. Body mass, body composition, and strength data were recorded pre- and postexpedition in addition to assessment of serum testosterone, LH, FSH, thyroid hormones, insulin-like growth factor 1 (IGF-1), and trace elements. Energy provision and energy expenditure were assessed at mid- and end-expedition. Daily energy provision was 6335 ± 149 kcal·day(-1). Estimated energy expenditure midexpedition was 5783 ± 1690 kcal·day(-1). Body mass and percentage body fat did not change between pre- and postexpedition. Total testosterone (nmol·L(-1)) (14.0 ± 4.9 vs. 17.3 ± 4.0, p = 0.006), calculated free testosterone (pmol·L(-1)) (288 ± 82 vs. 350 ± 70, p = 0.003), and sex hormone binding globulin (nmol·L(-1)) (33 ± 12 vs. 36 ± 11, p = 0.023) concentrations increased. LH and FSH remained unchanged. Thyroid stimulating hormone (TSH; IU·L(-1)) (2.1 ± 0.8 vs. 4.1 ± 2.1, p < 0.001) and free triiodothyronine (FT3; IU·L(-1)) (5.4 ± 0.4 vs. 6.1 ± 0.8, p < 0.001) increased while free thyroxine, IGF-1, and trace elements remained unchanged. Hand-grip strength was reduced postexpedition but static lift strength was maintained. Maintenance of body mass and nutritional status appeared to negate the central hypogonadism previously reported from polar expeditions. The elevated TSH and free FT3 were consistent with a previously reported "polar T3 syndrome"

    A Conceptual Measurement Model for eHealth Readiness: a Team Based Perspective.

    Despite the shift towards collaborative healthcare and the increase in the use of eHealth technologies, there does not currently exist a model for the measurement of eHealth readiness in interdisciplinary healthcare teams. This research aims to address this gap in the literature through the development of a three phase methodology incorporating qualitative and quantitative methods. We propose a conceptual measurement model consisting of operationalized themes affecting readiness across four factors: (i) Organizational Capabilities, (ii) Team Capabilities, (iii) Patient Capabilities, and (iv) Technology Capabilities. The creation of this model will allow for the measurement of the readiness of interdisciplinary healthcare teams to use eHealth technologies to improve patient outcomes

    Biodegradation of the Alkaline Cellulose Degradation Products Generated during Radioactive Waste Disposal.

    The anoxic, alkaline hydrolysis of cellulosic materials generates a range of cellulose degradation products (CDP) including α and β forms of isosaccharinic acid (ISA) and is expected to occur in radioactive waste disposal sites receiving intermediate level radioactive wastes. The generation of ISA's is of particular relevance to the disposal of these wastes since they are able to form complexes with radioelements such as Pu enhancing their migration. This study demonstrates that microbial communities present in near-surface anoxic sediments are able to degrade CDP including both forms of ISA via iron reduction, sulphate reduction and methanogenesis, without any prior exposure to these substrates. No significant difference (n = 6, p = 0.118) in α and β ISA degradation rates were seen under either iron reducing, sulphate reducing or methanogenic conditions, giving an overall mean degradation rate of 4.7×10−2 hr−1 (SE±2.9×10−3). These results suggest that a radioactive waste disposal site is likely to be colonised by organisms able to degrade CDP and associated ISA's during the construction and operational phase of the facility
